**About Us:**
Authentic Home Care was established in 2010 to provide IHSS and personal care services to the Somali community in Denver. Since then, the agency has expanded throughout multiple counties. AHC currently has the following programs: IHSS Medicaid Home Care Services; HCBS Medicaid Home Care Services; Pediatric Personal Care Services; Medicaid Home Health Services; and Medicare Home Health Services. This position will specifically oversee the Medicare and Medicaid Home Health Services but will be expected to understand and assist with all agency programs when necessary.

**Position Overview:**
We are seeking an experienced and dedicated Clinical Manager (Director of Nursing - DON) to lead our nursing team and ensure the highest quality of patient care. The DON will be responsible for overseeing the clinical operations of our facility, implementing nursing policies, and ensuring compliance with regulatory standards. This position requires strong leadership skills, a commitment to excellence in patient care, and the ability to foster a collaborative team environment.

**Key Responsibilities:**
- **Leadership and Management:**

- Directly supervise nursing staff, providing guidance, support, and performance evaluations.
- Facilitate ongoing staff training and development to enhance nursing skills and clinical knowledge.
- Promote a positive and healthy work environment through effective communication and team-building initiatives.
- Review and contribute to Quality Assurance/Program Improvement (QAPI) Plan, including monitoring appropriate data indicators and overall evaluation of the appropriateness and effectiveness of the Home Health services.

**Clinical Oversight:**
- Ensure compliance with all nursing care standards, policies, and procedures, and uphold the highest quality of patient care. - Monitor patient outcomes and implement strategies for improvement and quality assurance.
- Collaborate with interdisciplinary teams to develop and evaluate individualized care plans.

**Regulatory Compliance:**
- Stay current with state and federal regulations, accreditation standards, and best practices for nursing care.
- Prepare for and participate in regulatory surveys and audits, addressing any findings in a timely manner.
- Ensure proper documentation and record-keeping in accordance with legal and organizational requirements.

**Patient Care Management:**
- Establish efficient workflow from referral to discharge of patients.
- Accept and review referrals for patient care. Oversee completion of intakes for new patients.
- Monitor compliance and timely filing of documentation, including visit notes, progress notes, OASIS, etc.,
- Coordinate patient care and supervise clinical personnel per regulations.
- Continually assess patient needs and satisfaction.
- Evaluate the development, implementation, and updates of individualized plans of care.
- Provide on-call services to patients, including shift coverage if needed.
- Advocate for patients and families, providing education and resources as needed.
- Address patient and family concerns promptly and compassionately

**Budgeting and Resources:**
- Assist in managing departmental budgets, including staffing, supplies, and equipment needs.
- Develop and monitor cost-effective practices while maintaining high-quality care standards.

**Qualifications:**

- Must be available during all operating hours.- Must be available to provide on-call services outside of operating hours.
- Bachelor’s degree in Nursing (BSN) required; Master’s degree preferred.
- Current and valid Registered Nurse (RN) license.
- Previous experience in a clinical leadership role; a minimum of 3 years’ experience in nursing, with 2 years’ experience in nursing management.
- Excellent interpersonal, communication, and organizational skills.
- Proven ability to lead and inspire a diverse team of healthcare professionals.
- Able to demonstrate knowledge of federal, state, and local laws and regulatory guidelines that govern the operation of a Home Health Agency.
- Must be able to understand Electronic Medical Record systems in order to monitor compliance, submit OASIS documents, and train clinical personnel on its functions.
